The promotion spans more than 33 calendar days, so you don’t need a perfect daily streak from the start. However, you must record 33 eligible attendance days by the time it closes, which means missing too many days will put the Medical Support Box out of reach.
Missing a day doesn’t reset the days you’ve already accumulated.

Attendance resets daily at midnight PDT. That is the event’s own daily boundary, regardless of the time you play locally. Logging in on opposite sides of that reset can count as two different event days; logging in twice within the same reset window does not create an extra attendance day.
The event ends November 23 at 10:59 PM PST, after the 33-day attendance reward has been available to players who started early enough.

Yes. Nexon places Nell’s Medical Support Box on the Tactician Medical Support attendance track at 33 accumulated login days. It is an attendance reward rather than a purchased item from the Halloween Event Pass.
The First Descendant also runs separate October missions and character-overhaul events. Those involve objectives such as Onslaught, Sigma Sector and descendant-specific kills, while this particular reward is obtained by recording your logins.
The First Descendant Nell Medical Support Skin: do missions count?
No. The Medical Support Box is awarded at the 33-day accumulated attendance milestone. Event missions involving Freyna, Gley and Enzo have their own rewards, and running those missions repeatedly won’t add extra attendance days. You can play both event tracks together, but Nell’s attendance total increases through eligible daily logins.
What to do alongside your daily logins
If you’re returning for the October update, the Descendant Overhaul missions provide extra rewards for Freyna, Gley and Enzo. The current event includes Archive Shards, module rewards and External Components Selection Boxes from its activity goals.
You can work on those tasks while collecting the Medical Support attendance milestones. Just remember that progress in an overhaul mission doesn’t replace an attendance day for Nell’s Medical Support Box.
