Cyberpunk 2077 modded save not loading after moving between PC and console is often a mod compatibility problem. CD Projekt RED says a transferred save needs the same enabled mods on both platforms. If even one required mod is missing or incompatible with the destination, the saved game may fail to load correctly.
Update 2.5 now has mod.io integration and a cog icon beside modded saves in Load Game. Check the mod list before deleting anything: a cloud transfer and a mod subscription are related features, but they do not guarantee that every PC modification works on PS5 or Xbox.

Open Load Game and look for the mod.io cog icon beside the save. CDPR says any save created while mods were enabled is marked as modded. The icon makes it easier to separate that file from an older unmodified save before testing.
Update 2.5 also groups saves by playthrough, so expand the correct character group first. If the file has not appeared at all, follow the cross-progression account and cloud-upload checks rather than treating it as a failed load.

On the original system, open Mods and check the Installed section for enabled items. On the destination platform, log into the same CD PROJEKT RED account, connect mod.io and inspect Installed there as well. CDPR states that compatibility requires the same set of enabled mods across platforms.
Subscriptions sync through your linked account, but unsupported console mods are ignored and won’t appear there. For a Cyberpunk 2077 modded save not loading on PS5 or Xbox, inspect the missing file’s platform tag first. A PC mod may therefore be subscribed but unusable on PS5 or Xbox. Check each listing for its Available on Consoles tag.
Cyberpunk 2077 modded save not loading: Install required mods while online
The official workflow is to open Mods, connect your CDPR account to mod.io, subscribe to each compatible mod, restart the game and verify its checkbox is selected under Installed. Repeat on the target platform for every required supported mod.
A modded save can load offline only if the necessary mods have already been installed on that device. You cannot install missing mods offline. Reconnect to the internet first instead of repeatedly opening the save without its dependencies.

If the mod has no console release, CDPR warns that the transferred save may not load correctly. For a Cyberpunk 2077 modded save not loading because its PC mod is unsupported on console, the safest alternative is an earlier clean save, if you have one. Keep the working original on PC until you’ve tested an alternative. Do not delete the original or overwrite a working PC file simply to force the transfer.
Disabling a mod on an existing modded save doesn’t guarantee that its effects, dependencies or altered data disappear. A newly saved character may still carry the modded designation. Treat experiments on a backup as tests, not guaranteed repairs.
Do modded saves disable trophies and achievements?
On PS5 and Xbox, CDPR disables trophies and achievements while playing from a modded save. The cog icon is the visual warning in Load Game. Unsubscribing from a mod does not automatically prove that an already marked save becomes eligible again.
Keep a separate unmodded save for achievement hunting before enabling anything in the mod browser. If you already have a clean save, test that instead of overwriting it with a troubleshooting attempt.
Cyberpunk 2077 modded save not loading: Other reasons a cloud save fails
Check that both copies of Cyberpunk 2077 are patched and that the destination owns any content the save requires. A save made with Phantom Liberty installed cannot necessarily load in a base-game-only edition without the expansion. Also check account linking and the cloud icon on the source save.
