Diablo Immortal Made to Suffer: Everything New

Diablo Immortal Made to Suffer

Diablo Immortal Made to Suffer is the September 2026 major update that continues Nation of Agony, opens the High Ward, adds City of Pain and Twisting Torments, introduces two Legendary Gems, and changes several long-running progression systems.

The update is much broader than a single story chapter. Permanent Lut Gholein content arrives alongside a limited event, a weekly zone activity, Legendary Gem changes, Normal Gem progression, Hero’s Journey chapters, Battleground revisions and a sizeable quality-of-life pass.

Diablo Immortal Made to Suffer: Made to Suffer story, level requirement and High Ward

The new main quest requires level 60 and continues Nation of Agony in Lut Gholein. Blizzard warns that starting it before completing The Taking and Bloodied Jewel can reveal story spoilers, but those two quests are not listed as mandatory access requirements. The High Ward opens as a new district surrounding Sultan Jerhyn’s palace. It includes new bounties, objectives and wanted monsters, giving the city a new permanent exploration area rather than a one-use story backdrop. That matters because Diablo Immortal Made to Suffer mixes permanent additions with content that only runs during specific dates.

Diablo Immortal Made to Suffer: City of Pain and Twisting Torments

City of Pain runs from September 17 through September 30. The new Main Story, Lut Gholein activities, Battlegrounds, dungeons and other eligible content can award event tokens while players continue normal progression. Players can earn up to 1,300 City of Pain Tokens each week. Those tokens are spent through the Treasure of Hope, where Blizzard specifically names Sacred Chests and Legendary Equipment among the rewards.

Twisting Torments is a Tuesday event in the Rocky Waste outside Lut Gholein. Either Andariel or Duriel invades for the full day, and players clear three altars before a Lieutenant appears. For returning players, Diablo Immortal Made to Suffer therefore has value beyond the opening event window, particularly through High Ward, the new gems and progression changes.

Enigma Latera, Prong Crag and progression changes

Enigma Latera is the new 5-Star gem and Prong Crag is the new 2-Star gem. Both enter the standard pool, while Enigma Latera also receives a featured pool and temporary Rank 10 trial. Normal Gems gain Progressive Upgrades beginning at Rank 6. This gives players smaller stat increases at intermediate breakpoints rather than leaving the gem unchanged until the next complete rank is finished.

Chapters 5 and 6 unlock in sequence. Chapter 5 cannot progress until Chapter 4 is complete, while Chapter 6 remains locked from progress until Chapter 5 is finished. Diablo Immortal Made to Suffer also makes Lut Gholein the centre of several overlapping systems instead of scattering every new feature across unrelated zones.

Battlegrounds, Sanctum and other quality-of-life changes

Battlegrounds return to the Nephalem visual theme, remove Small Demons and Demon Gates, retain the wider roads and adapt the central mechanic around the Sacred Guardian. Seasons also shorten from 10 weeks to eight. Iben Fahd’s Sanctum gains extra chest opportunities after the existing daily openings. Player inspection expands, Main Quest dungeon difficulty can be lowered after repeated defeats, and Raid the Vault’s Hidden Passage is easier to activate.

Blizzard says 100 Affix effects have been reworked, lighting and volumetric fog have improved across more than 10 zones, and terrain textures, water reflections and visual-effect clutter also receive changes. Because Diablo Immortal Made to Suffer changes story, events and account progression together, it is worth separating permanent systems from launch-period activities.

Diablo Immortal Made to Suffer is best understood as a system-wide update rather than one new quest. The High Ward and story are permanent, City of Pain is date-limited, Twisting Torments becomes a weekly target, and the gem and progression changes remain relevant long after launch week.

The update also introduces the Bloodied Jewel replay through the Adventurer’s Book at Cain’s Workshop and reorganizes the Event tab with categories and sub-tabs. Those additions are smaller search targets but reinforce how much of the game is being touched by the same release. Diablo Immortal Made to Suffer keeps giving players things to do after City of Pain ends, which is why the update is broader than a temporary event patch.

Made to Suffer also changes how players approach the update chronologically. The main quest becomes available once the level requirement is met, while City of Pain, the Enigma Latera trial and the featured gem pool each follow their own dates. That means patch availability and event availability are not identical.

The update also adds replay support for Bloodied Jewel through the Adventurer’s Book at Cain’s Workshop. That is useful for returning players because it gives them an in-game way to refresh the Nation of Agony story before moving into the new chapter. Seen as a whole, Diablo Immortal Made to Suffer links its strongest new content through Lut Gholein, weekly activities and longer-term progression.

Blizzard lists these changes in the official Made to Suffer update notes.
