Diablo 4 Horadric Items are a new type of Legendary equipment being tested ahead of Season 16, and their main attraction is how they interact with Talisman set bonuses. A Horadric affix on jewelry can help activate a set bonus with fewer equipped Charms, while suitable armor and weapons can add further effects when the required set is active. Blizzard has confirmed the rules for each equipment slot in its 3.3.0 PTR announcement.
For anyone who enjoys experimenting with equipment, Diablo 4 Horadric Items are a more interesting change than a simple increase to a damage number. A new piece of jewelry could free a Charm slot for something else, potentially giving you greater flexibility across multiple sets. Start with matching jewellery and one Charm from the same set to activate the two-piece bonus.
How Diablo 4 Horadric Items work with Talisman sets
Blizzard calls these Legendary items with Horadric affixes tied to particular Talisman sets. That distinction is crucial. Diablo 4 Horadric Items are not the same as crafting materials, a recipe category or anything you happen to put into the Horadric Cube. Their special characteristic is the relationship between an affix on equipped gear and a defined Talisman set. The effects differ according to whether that affix belongs to jewelry, armor or a weapon.

The Death Cult of Ullo Sar introduces these items. In the 3.3.0 PTR, Mrak’s seasonal item boost can grant Horadric Items directly so you can test their set effects.
Players coming back to the Talisman system can start with our Diablo 4 Talisman guide, which covers Charms, Seals and slots, or the more focused set bonuses guide. Understanding those basics makes the effect of Diablo 4 Horadric Items much easier to appreciate, particularly when a set would otherwise require another Charm.
Why Horadric jewelry matters for set bonuses
With Diablo 4 Horadric Items, a jewelry item carrying the appropriate affix counts as one additional piece for the specified Talisman set. According to Blizzard, that means a corresponding single Charm plus the matching Horadric jewelry can activate a two-piece set bonus. The extra contribution is tied to the specified set, so do not read this as a universal extra piece for every Talisman bonus on your character.
Think of it as a way to change the amount of space required to obtain a particular effect, rather than a guarantee that every set becomes stronger. Freeing a Charm slot could let you try another useful option, but the actual gain depends on the item, its affix and the other Charms you have equipped. Diablo 4 Horadric Items therefore give you more flexibility when arranging Charms and activating set bonuses.
Blizzard compares Horadric jewellery with the Seal of the Diamond Mind. For background, our guides to using Seals and unlocking Charm slots explain why slot availability matters. Those existing instructions should not be mistaken for a confirmed recipe that creates Horadric jewelry, however; Blizzard has not supplied such a recipe in the tested announcement.
What Horadric armor and weapons add
Among Diablo 4 Horadric Items, armor and weapons work differently from jewelry. Their Horadric affixes can grant additional effects when the character has at least the relevant two-piece Talisman bonus active. Blizzard gives broad examples including extra skill ranks, increased damage and defensive benefits. Check the set named on the Horadric affix, then activate its matching two-piece bonus to enable that effect.
Meet the named set requirement first, then check what the equipped armour or weapon adds. If a conditional affix does nothing in a test character, it may be because the matching set threshold is missing rather than because the item is broken. When testing Diablo 4 Horadric Items, reading the actual set name and active bonus is more reliable than assuming that any two Charms will do.
It also helps to keep the existing Charm controls straight. The guide to equipping Charms explains the ordinary slot interaction, while changing Charms addresses adjustments to an existing selection. Players interested in maximum room for combinations can consult how many Charm slots are available. Those mechanics are the foundation; the new affixes are the additional layer being tested.
How to test Diablo 4 Horadric Items with matching Charms
To test Diablo 4 Horadric Items, equip a Charm from the named Talisman set, then equip matching Horadric jewellery. The jewellery counts as another set piece, activating the two-piece bonus. Add matching armour or a weapon next to test its effect with that threshold active.
In the Season 16 PTR, Mrak grants Horadric Items through the Seasonal Items boost. Visit him near a main city Waypoint, collect the test pieces and check the set named on each affix.
For an accurate comparison, note the item tooltip, its Talisman set, your equipped Charms and which set bonus is active. Swap one piece and see whether the corresponding effect changes. Our PTR Build Testing guide covers the vendor setup.
How to check a Horadric Item bonus
Diablo 4 Horadric Items work through named Talisman sets: matching jewellery contributes one extra set piece, and compatible armour or weapons add effects when the required set bonus is active. Test each slot with the corresponding Charm equipped.
Blizzard’s 3.3.0 PTR announcement confirms that jewellery adds one matching set piece while qualifying armour and weapons provide additional effects after the relevant set threshold is active.
With Diablo 4 Horadric Items, start by checking the Talisman set named on the affix, then test the result with the matching set bonus active. Compare the active set bonus before and after equipping the item to see exactly what changes.
Diablo 4 Horadric Items not working in the Season 16 PTR
Blizzard’s 27 October 3.3.0 PTR issue list confirms that some Diablo 4 Horadric Items do not currently activate their intended bonuses. For the Warlock Flesh of Abaddon, bonus damage while in Demonform may fail to apply until you leave Demonform and re-enter it. That is the one specific workaround Blizzard describes for this group of problems.
The Warlock’s Fulcrum of Mephis interaction has a different bug. Its chance to convert slain enemies into Lesser Demons only works when the character has the actual Fulcrum of Mephis set bonus. Receiving the matching set benefit from a Horadric Item does not trigger it in this test. A Paladin Horadric damage bonus against Weakened enemies also fails to apply, and Blizzard has not provided an in-game fix.
Two other issues concern Talisman sets rather than Diablo 4 Horadric Items themselves. The Druid’s Song of the Old Mountain five-piece Petrify explosion remains at 200% instead of the updated 300% value. The Rogue’s Spellbound Steel five-piece effect does not increase Shadow Imbuement when used by Shadow Clone. Neither is a reliable basis for judging the finished Season 16 damage values.
These effects are bugged in the 3.3.0 PTR. If you also find your War Plans points missing on an alt, the zone-change workaround is covered separately. If you’re testing Diablo 4 Horadric Items now, note the character, matching set, active bonus and weapon or jewellery you have equipped before reporting a new issue.
